John Rambo is an aging Vietnam veteran who lives on his late father's horse ranch in Arizona. He has become a father figure to Gabriela, the teenage granddaughter of his housekeeper Maria. When Gabriela crosses into Mexico to meet her absent father, a cartel kidnaps her and forces her into sex work. Rambo travels across the border, rescues her too late, and begins a violent hunt for the men who hurt her.
The film uses Mexican characters because Gabriela's family and the cartel drive the story. Rambo protects Gabriela and Maria while he struggles with old war trauma. The story focuses on family loyalty, human trafficking, revenge, and the fear that violence has become Rambo's only way to live. It shows the cartel as cruel criminals and gives little space to ordinary life outside that criminal world. Its tone is grim and extremely bloody, and the final fight turns Rambo's ranch into a trap-filled war zone.
