Iris is a grieving mother who returns to a forest in Big Sur after her young son dies in an accident. She is thinking about ending her own life when a friendly stranger named Richard speaks to her. Richard then attacks her, injects her with a drug, and gives her about twenty minutes before her body stops moving. The film becomes a tense survival chase as Iris hides, crawls, and fights while the drug spreads.
Richard is a serial killer who enjoys watching women suffer, and he uses charm to control people before he hurts them. Iris slowly finds a reason to live again, even while grief and the drug leave her weak and nearly unable to speak. Bill, an older man, gives her a brief chance to hide, while a police officer becomes another victim of Richard's violence. The story ends with Iris fighting back, killing Richard, and walking away after she chooses life over despair.
